You have to upload your ID (front and back) and Medical Documentation of your qualifying condition to our patient portal before time of visit. If you do not have to proper documentation we cannot approve you. If you are scheduling for same day make sure you have both your ID and Medial Documentation ready to upload before you access our same day appointment link.
